I am fully aware that martial arts training, which is a contact sport, carries a certain physical risk such as but not limited to:

Bumps & bruises.
Dislocations
Minor Contusions
Concussion
Sprains and strains
Muscle / Tendon / Ligament tears.

And other injuries not listed above.

I recognise Escapology BJJ do all that is reasonable in minimising and preventing risks of injury in normal scheduled practice including completion of PAR-Q's, rules, safety protocols, staff training, gym checks, curriculum and other procedures not listed.

I have been duly informed and accept the associated risks and agree to comply with the rules and safety protocols that are in place. I will conduct myself in a manner that does not put myself or others at risk of injury. I will notify the coaching staff if I have any injury or condition which might affect my training.
